Year End Coach and Staff Feedback Survey

Attention all Wolverines and parents!  Once again, we are coming to the end of a wonderful year of girls' hockey in Wilmot.  To help us provide the very best experience for our players, please take a moment to fill out a short survey:

Year End Coach and Staff Feedback Form




This form can be used to provide feedback about your head coach, assistant coaches, trainers, on-ice helpers, or any team/organization staff that you had contact with this season. Players who are old enough to participate online with parental permission are also encouraged to complete the feedback form.

Please be advised that ALL FEEDBACK IS VERY STRICTLY CONFIDENTIAL.  Good or bad, no coach or staff member will ever know who provided the feedback that they receive.  Even if you choose to provide your name and contact information on your survey, this information will never be shared with anyone, and will only be used if you have asked us to contact you.  The ratings and comments from all surveys will be lumped together to give coaches and staff an all-round review of their performance this season.

If you have any questions or concerns about how to provide feedback or how this information will be used, please don't hesitate to contact the WGHA President, Yolanda Galambos.  If you have any difficulty with the online survey or submitting your results, please contact WGHA Webmaster, Stacey Dineen.
